Intake / Health History
On your first visit to our clinic you will be asked to fill out our intake form, or you can submit it online prior to your appointment by clicking here. This form will ask you your general information and health history. The purpose for taking your health history is to accumulate as much information about you and your condition as possible before you consult with the doctor. This information is vital to help us better serve you.

Consultation
To help put together the best possible treatment and payment plans for each individual, there will then be a consultation between you and the doctor in which he will go over the information you have provided and ask any extra questions that may be needed.

Chiropractic Examination
After Dr. Nowicki has obtained all the information he needs, he will then proceed with a physical exam, any necessary x-ray imaging and treatment.

Report of Findings
You will then ask to schedule an appointment for the next day in order to receive the report of findings and discuss your treatment plan. If for some reason the doctor feels he will not be able to help you, you will be referred to someone who will. Otherwise, you are ready for your first adjustment!
